Caregiver coordination is designed to provide information, resources, education, training and benefits counseling to informal caregivers such as family and friends. Caregiver coordination focuses on those who take care of people age 60 and over. It’s focus is to provide a variety of support that will temporarily relieve caregiver responsibilities. As a caregiver, you will learn about and gain access to programs and services, and you will be provided with educational opportunities to assist you in making decisions and solving problems relating to you caregiver role.
